Care Co-ordinator forum
Care Co-ordinator education and support forum open to care Co-ordinators working in Wandsworth and Merton.
This educational forum has been specifically organised for Care co-ordinators who are employed in General Practice and aims to bring an awareness of this relatively new dynamic role, which focuses on delivering personalised care; that is evolving and maturing in the health and care system.
The forum is interactive and currently runs every 6-8 weeks from 12.00pm – 1.30 pm via MS Teams and provides learning opportunities, supports retention and a safe space to network with peers across Primary Care Networks in SW London.

Demonstrate the impact of Care co-ordinators in Primary Care
Identify CPD needs and opportunities
Establish a network with local peers
Create a safe space for peer support
Interactive Question and Answer session with leading Industry Professionals
Share ideas, learning and reflect on safe practice
Understand the boundaries of Care co-ordinators – the role is non-clinical
Focus on personal development to manage wellbeing.
