Care Co-ordinator forum

This educational forum has been specifically organised for Care co-ordinators who are employed in General Practice and aims to bring an awareness of this relatively new dynamic role, which focuses on delivering personalised care; that is evolving and maturing in the health and care system.

The forum is interactive and currently runs every 6-8 weeks from 12.00pm – 1.30 pm via MS Teams and provides learning opportunities, supports retention and a safe space to network with peers across Primary Care Networks in SW London.

The purpose of the forum is to:

• Demonstrate the impact of Care co-ordinators in Primary Care

• Identify CPD needs and opportunities 

• Establish a network with local peers

• Create a safe space for peer support

• Interactive Question and Answer session with leading Industry Professionals

• Share ideas, learning and reflect on safe practice

• Understand the boundaries of Care co-ordinators – the role is non-clinical
